Job Summary

The Population Health Specialist Embedded at Duke University will play a crucial role in developing and implementing comprehensive patient plans to ensure optimal medical care, therapy, and training services for patients with complex, chronic health conditions.

Key Responsibilities
  • Assess patient conditions, locate appropriate treatment and resources, and ensure continuity of care.
  • Provide individual counseling sessions concerning rehabilitation treatment and health maintenance.
  • Document interventions within medical record systems to collaborate with healthcare providers and monitor treatment programs.
  • Review patient data related to disabilities or medical limitations and work with interdisciplinary teams to promote a healthy context or social environment.
  • Participate in multi-disciplinary supporting local partnerships to broaden the local response to health inequalities and advocate for patient-centered care.
  • Conduct community, telephone, and practice encounters with patients and other care management team members to identify care plans, barriers, and goals.
  • Follow-up with patients and providers on identified healthcare needs and identify possible resources to address those concerns.
  • Facilitate and manage referrals from referral specialists, providers, and other care management staff to ensure that identified red flags and healthcare needs of patients are addressed.
  • Provide individual consults to patients on health education issues and develop the health awareness of individuals, as well as groups and organizations, empowering them to make better health choices.
Requirements
  • Bachelor's degree in business, behavioral/social sciences, public health, or a related population health field.
  • Three years of experience in a business, behavioral/social sciences, public health, or related population health field.
  • Sales and marketing background, along with professional experience in social work, disease management, and experience working directly with physicians and advanced practice providers is strongly preferred.
