About the Role

We are seeking a highly motivated and experienced Population Health Resource Associate to join our team at Duke Health. As a key member of our pediatric population health team, you will play a critical role in supporting the health and well-being of our patients and their families.

Key Responsibilities
  • Provide outreach and care management support to patients and their families, ensuring they receive the necessary resources and services to achieve optimal health outcomes.
  • Educate clients on available community resources and services, empowering them to make informed decisions about their care.
  • Manage an active caseload, balancing multiple tasks and responsibilities while maintaining a high level of productivity and performance.
  • Review and update person-centered care plans to measure progress and patient outcomes, ensuring that care is tailored to meet the unique needs of each individual.
  • Collaborate with a multi-disciplinary team to provide comprehensive care and support to patients and their families.
  • Complete required documentation within the expected program timeframe, ensuring accurate and timely reporting of patient outcomes and progress.
  • Utilize electronic medical records and document according to agency standards, maintaining confidentiality and adhering to HIPAA guidelines.
Requirements
  • High school degree or equivalent, with strong communications and organizational skills.
  • Two years of experience in customer service, with a focus on patient or public contact.
  • Active user of electronic medical records software, with a strong understanding of data analysis and reporting.
  • Ability to work in a fast-paced environment, managing multiple tasks and responsibilities while maintaining a high level of productivity and performance.
  • Strong communication and interpersonal skills, with the ability to build effective relationships with patients, families, and healthcare providers.
