Job Summary:The Population Health Care Manager is responsible for delivering clinical expertise to manage health care needs of specific patient populations across the continuum of care with a goal of improving patient health outcomes and reducing unnecessary utilization and cost.
Key Responsibilities:- Deliver clinical expertise to manage health care needs of specific patient populations
- Develop and implement care plans to improve patient health outcomes and reduce unnecessary utilization and cost
- Collaborate with interdisciplinary teams to optimize clinical outcomes
- Provide education and support to patients and families to promote self-management and health behavior change
- Monitor quality and effectiveness of interventions and make data-driven decisions to improve care
- Bachelor's degree in Nursing or Master's degree in a related field
- 3 years of relevant clinical experience
- Current licensure as a Registered Nurse (RN) or Licensed Clinical Social Worker (LCSW)
- Case management certification (ACM, CCM, or ANCC)
