Job Title: Population Health Care Manager

Job Summary:

The Population Health Care Manager is responsible for delivering clinical expertise to manage health care needs of specific patient populations across the continuum of care with a goal of improving patient health outcomes and reducing unnecessary utilization and cost.

Key Responsibilities:
  • Deliver clinical expertise to manage health care needs of specific patient populations
  • Develop and implement care plans to improve patient health outcomes and reduce unnecessary utilization and cost
  • Collaborate with interdisciplinary teams to optimize clinical outcomes
  • Provide education and support to patients and families to promote self-management and health behavior change
  • Monitor quality and effectiveness of interventions and make data-driven decisions to improve care
Requirements:
  • Bachelor's degree in Nursing or Master's degree in a related field
  • 3 years of relevant clinical experience
  • Current licensure as a Registered Nurse (RN) or Licensed Clinical Social Worker (LCSW)
  • Case management certification (ACM, CCM, or ANCC)
