Population Health Resource Specialist

Duke Health is seeking a skilled Population Health Resource Specialist to join our team. As a key member of our care management team, you will work closely with patients, families, and healthcare providers to ensure seamless care coordination and support.

Key Responsibilities:

  • Provide outreach and care management support to patients with intellectual and developmental disabilities (I/DD)
  • Educate clients on available community resources and support services
  • Collaborate with care managers and specialists to develop and implement personalized care plans
  • Review and update person-centered care plans to measure progress and patient outcomes
  • Act as a liaison between patients, families, and healthcare providers to ensure effective communication and care coordination

Requirements:

  • Two years of experience in customer service, healthcare, or a related field
  • Strong communication and organizational skills
  • Ability to work in a fast-paced environment and manage multiple tasks simultaneously
  • Proficient in electronic medical records software
  • Ability to maintain confidentiality and handle sensitive information with discretion
