Job Title: Population Health Resource Associate Pediatric Family Advocate

Job Summary:

Duke Health is seeking a Population Health Resource Associate Pediatric Family Advocate to join our team. This role will work with our care managers and specialists to provide support and advocacy to patients and their families navigating intellectual and developmental disabilities diagnosis.

Key Responsibilities:

  • Provide outreach and care management support to patients
  • Educate clients on available community resources
  • Manage an active caseload while balancing multiple tasks
  • Review and update person-centered care plans to measure progress and patient outcomes
  • Collaborate with a multi-disciplinary team
  • Complete required documentation within expected program timeframe
  • Navigate electronic medical records and document according to agency standards

Requirements:

  • Two years of customer service experience
  • Healthcare-related experience preferred
  • Ability to manage multiple tasks and projects simultaneously
  • Proficient in computer software tools and data files
  • Comfortable with continuous change and self-initiating
