POSITION OVERVIEW:
The Senior Nursing Coordinator, under the guidance of the Director of Nursing and the clinical care team, plays a crucial role within the nursing and leadership teams at CCI Health Services. As a key member of the health center leadership triad, the Senior Nursing Coordinator engages in administrative and organizational leadership efforts to enhance efficiency and ensure optimal quality of care. This role involves supporting the CCI team in delivering high-quality, evidence-based patient care and managing high-risk patients with chronic conditions, focusing on health education, condition management, and self-advocacy to achieve improved patient and financial outcomes.
KEY FUNCTIONS AND RESPONSIBILITIES:
As part of the integrated care team, the Senior Nursing Coordinator is responsible for:
- Leading the health center by exemplifying and executing routine nursing care, care coordination, and management duties to enhance patient care, maintaining high standards of quality and safety for patients across all age groups, including:
- Building strong relationships with patients
- Facilitating clinical huddles, pre-visit planning, and identifying complex care patients
- Conducting patient appointments in collaboration with clinicians, including gathering health histories, assessing needs, and identifying care gaps
- Providing medical advice and discharge instructions supported by evidence-based resources for various patient populations
- Reviewing and reconciling medical and vaccination records, completing necessary documentation for patients and other healthcare entities
- Reconciling medication histories and advising patients on medication use, benefits, risks, and potential side effects
- Coordinating clinical care and transitions, supporting the referral process
- Educating patients on chronic disease management and collaborating with care teams for appropriate follow-up
- Implementing treatment plans as directed and evaluating outcomes, including administering tests, vaccinations, and medications
- Providing information in the patient's preferred language, utilizing interpreters as needed
- Managing a panel of care management patients by developing care plans and assessing self-management and barriers to care
- Collaborating with population health and leading monthly care management meetings
- Serving as a leader within the nursing department and being an essential part of the leadership triad, which includes:
- Establishing and maintaining policies, procedures, workflows, and clinical guidelines
- Collaborating with health center leadership to address concerns
- Participating in quality assurance and improvement activities to meet organizational goals
- Supervising nursing staff, including assigning duties and providing training and education
- Overseeing inventory and ordering of medical supplies and equipment
- Planning and attending meetings, health fairs, events, and training sessions
- Continuously seeking opportunities for self-education to enhance knowledge and maintain compliance with best practices
- Utilizing the Electronic Medical Record system to maintain accurate patient records
- Delivering exceptional customer service while ensuring HIPAA compliance and cultural sensitivity
- Adhering to the policies and procedures of CCI Health Services
- Performing additional duties as assigned by the Director of Nursing and other leadership members
- Active RN license from the State of Maryland Board of Nursing or any state under the Nurse License Compact
- At least three (3) years of nursing experience, with leadership experience preferred
- Current BLS/CPR certification
- Experience with Electronic Medical Record documentation; familiarity with eClinicalWorks is a plus
- Prior supervisory experience is advantageous
- Fluency in English, with Spanish proficiency being a plus
