About the Role

We are seeking a highly skilled and dedicated Clinical Coordinator to join our team at TRC Total Renal Care Inc. As a Clinical Coordinator, you will play a vital role in ensuring the highest level of care for our patients with end-stage renal disease or chronic kidney disease.

Key Responsibilities
  • Coordinate patient care and services, including trend lab values, vital signs, and outcome management.
  • Ensure the safety, comfort, and well-being of patients by delivering high-quality care and services.
  • Train and mentor clinical team members to ensure they are equipped to provide exceptional patient care.
  • Collaborate with interdisciplinary teams to develop and implement patient care plans.
  • Monitor and report on patient outcomes and quality metrics.
Requirements
  • Current Registered Nurse (RN) license in the state of practice.
  • At least 18 months of registered nursing experience, preferably in dialysis or a related field.
  • Dialysis Charge RN readiness approval by clinical Nurse Manager or RN-licensed FA/GFA required.
  • Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN) preferred.
  • Basic computer skills and proficiency in MS Word and Outlook required.
