ROLE SUMMARY:
The Senior Registered Nurse (Lead RN) operates under the guidance of the Director of Nursing and collaborates closely with the clinical care team. This role is vital to the nursing and leadership teams at Community Clinic Inc. As a key member of the health center leadership triad, the Lead RN engages in administrative and organizational initiatives aimed at enhancing operational efficiency and ensuring the highest quality of care. Responsibilities include supporting the Community Clinic team in delivering superior, evidence-based patient care and managing high-risk patients with chronic conditions, focusing on health education, condition management, and patient self-advocacy to achieve better health and financial outcomes.
As part of the integrated care team, the Lead RN is responsible for the following KEY FUNCTIONS AND RESPONSIBILITIES:
  • Act as a leader within the health center by exemplifying and executing routine nursing care, care coordination, and management tasks to enhance patient care while maintaining high standards of quality and safety for a diverse patient population, which includes:
    • Building strong relationships with patients.
    • Leading clinical discussions, pre-visit planning, and identifying patients with complex care needs.
    • Conducting patient consultations in partnership with clinicians, gathering health histories, assessing needs, and identifying care gaps.
    • Providing informed medical advice and discharge instructions supported by evidence-based resources for various patient demographics.
    • Reviewing and reconciling medical records and completing necessary documentation for patients and healthcare entities.
    • Assessing medication histories and advising patients on medication use, benefits, risks, and potential side effects.
    • Coordinating clinical care transitions and facilitating the referral process.
    • Educating patients on chronic disease management, preventive health measures, and follow-up care as needed.
    • Implementing treatment plans as directed by the Director of Nursing or clinicians, evaluating outcomes, and managing point-of-care testing and medication administration.
    • Providing information in the patient's preferred language, utilizing interpreters as necessary.
    • Maintaining a caseload of care management patients by developing care plans and setting patient-centered goals while assessing barriers to care.
    • Collaborating with population health initiatives and leading monthly care management meetings with community health workers.
  • Serve as a leader within the nursing department at the health center, contributing to the leadership triad, which includes:
    • Developing and enforcing clinical policies, procedures, and guidelines.
    • Working with health center leadership to address and resolve operational concerns.
    • Participating in quality assurance and improvement initiatives to meet organizational goals.
    • Supervising nursing staff, including task assignment, training, and performance management.
    • Overseeing inventory management and procurement of medical supplies and equipment.
  • Plan and participate in meetings, health fairs, and training sessions as required.
  • Continuously seek opportunities for professional development to enhance knowledge and maintain compliance with best practices.
  • Utilize the Electronic Medical Record system to document patient information accurately and in accordance with accepted standards.
  • Deliver exceptional customer service while ensuring HIPAA compliance and cultural sensitivity.
  • Adhere to the policies and procedures of Community Clinic Inc.
  • Perform additional duties as assigned by the Director of Nursing and other leadership personnel.

MINIMUM QUALIFICATIONS:
  • Active RN license from the State of Maryland Board of Nursing or any state under the Nurse License Compact.
  • At least three (3) years of nursing experience, with leadership experience preferred.
  • Current BLS/CPR certification is mandatory.
  • Experience with Electronic Medical Records (EMR) documentation; familiarity with eClinicalWorks is advantageous.
  • Prior experience in a supervisory role is a plus.
  • Fluency in English, both written and spoken.
  • Spanish language skills are a plus.
