ROLE SUMMARY:
The Senior Registered Nurse (Lead RN) plays a crucial role under the guidance of the Director of Nursing and the clinical care team, contributing significantly to the nursing and leadership teams at Community Clinic Inc. As part of the health center leadership triad, the Lead RN engages in administrative and organizational leadership to enhance efficiency and ensure the highest quality of care. This position is vital in supporting the Community Clinic team in delivering exceptional, evidence-based patient care and managing high-risk patients with chronic conditions, focusing on health education, condition management, and self-advocacy to improve patient outcomes and financial sustainability.
KEY FUNCTIONS AND RESPONSIBILITIES:
  • Acts as a leader within the health center by demonstrating and executing routine nursing care, care coordination, and management duties to optimize patient care, maintaining high standards of quality and safety for a diverse patient population, including:
    • Building strong relationships with patients.
    • Leading clinical huddles, pre-visit planning, and identifying complex care patients.
    • Conducting patient appointments in collaboration with clinicians, gathering health histories, assessing needs, and identifying care gaps.
    • Providing informed medical advice and discharge instructions supported by evidence-based resources across various populations.
    • Reviewing and reconciling medical and vaccination records, completing necessary documentation for patients and other healthcare entities.
    • Managing medication histories and advising patients on medication use, benefits, risks, and potential side effects.
    • Coordinating clinical care and transitions, supporting the referral process.
    • Educating patients on chronic disease management, preventive care, and follow-up screenings as necessary.
    • Implementing treatment plans as directed by the Director of Nursing and evaluating outcomes, including administering tests, vaccinations, and medications.
    • Providing information in the patient's preferred language, utilizing interpreters as needed.
    • Maintaining a panel of care management patients by developing care plans and assessing barriers to care.
    • Collaborating with population health initiatives and leading monthly care management meetings.
  • Serves as a leader within the nursing department and is a key member of the leadership triad, which includes:
    • Developing and enforcing policies, procedures, and clinical guidelines.
    • Collaborating with health center leadership to address concerns and implement solutions.
    • Participating in quality assurance and improvement activities to meet organizational goals.
    • Supervising nursing staff, including hiring, training, and ongoing education.
    • Overseeing inventory and ordering of medical supplies and equipment.
  • Planning and attending meetings, health fairs, and training sessions.
  • Continuously seeking professional development opportunities to enhance knowledge and maintain compliance with best practices.
  • Utilizing Electronic Medical Records to document patient information accurately.
  • Providing exceptional customer service while adhering to HIPAA regulations and cultural sensitivity.
  • Operating within the policies and procedures of Community Clinic Inc.
  • Performing additional duties as assigned by the Director of Nursing and other leadership personnel.

MINIMUM QUALIFICATIONS:
  • Active RN license from the State of Maryland Board of Nursing or any state under the Nurse License Compact.
  • At least three (3) years of nursing experience, with leadership experience preferred.
  • Current BLS/CPR certification is required.
  • Experience with Electronic Medical Records (EMR) is essential; familiarity with eClinicalWorks is a plus.
  • Prior supervisory experience is advantageous.
  • Fluency in English, both written and spoken.
  • Spanish language skills are a plus.
