Job Summary

The Member Health Assessor plays a critical role in establishing effective relationships with members, with primary responsibility for conducting health risk assessments.

Key Responsibilities
  • Collaborate with the Integrated Care Coordination Team to ensure seamless care coordination.
  • Coordinate logistics to support care plan goals and interventions, addressing immediate member concerns as needed.
  • Verify member eligibility, previous enrollment history, demographics, and current health status prior to outreach.
  • Conduct assessments by gathering information from members, families, providers, and other stakeholders, as directed by members or their legally authorized representatives.
  • Contribute to care plan development and reporting, based on assessment outcomes, when needed.
  • Provide health education and wellness materials, as well as coaching, as appropriate.
  • Maintain accurate documentation within established protocols and guidelines.
  • Empower members by helping them feel informed, empowered, and supported by CareSource.
  • Identify opportunities for process improvement to enhance the member experience and streamline assessment and follow-up processes.
  • Perform other job duties as requested by management.
Requirements
  • High School Diploma or General Education Diploma (GED) is required.
  • Minimum of two (2) years of experience in community settings, working with at-risk populations, providing coordination of services, is preferred.
Competencies and Skills
  • Proficient in Microsoft Office, including Outlook, Word, and Excel.
  • Sensitivity to diverse cultures and experience working within different cultural contexts.
  • Excellent interpersonal skills and ability to work independently and within a team environment.
  • Ability to identify problems and opportunities and communicate effectively with management.
  • Developing knowledge of local, state, and federal healthcare laws and regulations and company policies regarding case management practices.
  • Demonstrate compassion, support, and collaboration with members and families.
  • Self-motivated and inquisitive with a comfort level asking pertinent questions.
  • Ability to work in a fast-paced environment and maintain composure during high-stress situations.
  • Ability to maintain confidentiality and act in the company's best interest.
  • Proven track record of demonstrating empathy and compassion for individuals.
  • Proven ability to improve processes to enhance the member experience.
