Job Overview

Position Summary: The role involves overseeing clinical information extraction from OASIS and various medical documentation. This includes the assignment of appropriate ICD-10 diagnosis codes to patient records following established protocols. The position also entails recommending modifications to OASIS based on clinical documentation and advising clinicians accordingly. Additionally, the role includes working with coding databases and verifying HHRG assignments while managing the clinical care services of the designated team.

Key Responsibilities:

  1. Guarantees the safe and high-quality delivery of home health services.
  2. Establishes and evaluates staffing patterns to reflect the productivity of both skilled and non-skilled personnel.
  3. Coordinates and supervises the quality of care provided by CareGivers, Inc. clinical and support staff.
  4. Acts as a liaison between the agency and medical offices, hospitals, and other external organizations to address client issues, accept medical orders, process referrals, and conduct necessary follow-ups.
  5. Assigns registered nurses, physical therapists, or speech therapists for initial assessments as required.
  6. Develops methods for interdisciplinary care coordination.
  7. Oversees staffing patterns, missed visits, and all scheduling-related functions.
  8. Monitors overtime, inappropriate staffing levels, and redline occurrences.
  9. Reviews all care authorizations to ensure proper routing and utilization of prescribed units.
  10. Participates in the recruitment and orientation of new staff members.
  11. Maintains up-to-date knowledge of healthcare services provided in home settings, as well as current Medicare, Medicaid, insurance, and pre-authorization processes.

Qualifications: Must be a graduate of an accredited nursing program, with a BSN preferred. A minimum of two years of experience in home health OASIS review is required, along with five years of experience in home health and/or hospice preferred. Certification in home health coding (HCS-D or BCHH-C in ICD-10) and OASIS (COS-C or HCS-O) is necessary. Experience in quality assurance or case management is advantageous.
