Role Description

Key Responsibilities:

The main objective of the Service Coordination Specialist is to facilitate service coordination for individuals with Autism and Developmental Disabilities, supporting their aspirations for a fulfilling life. Once individuals are deemed eligible for services, the Specialist is tasked with guiding them through the service system, accessing necessary supports, providing vital information to individuals and families, collaborating with team members, and fostering positive community connections. Services are provided throughout the individual's life, ensuring ongoing support.

Core Duties:

Evaluation.

Conduct initial and continuous assessments of the strengths and needs of the individuals served.

Strategic Planning.

Work alongside individuals and team members to create detailed plans that highlight strengths, support requirements, and objectives for those served.

Resource Coordination.

Identify and connect individuals with necessary services and supports tailored to their unique needs and goals.

Progress Monitoring & Support.

Regularly assess individual progress and provide continuous support, guidance, and motivation to ensure adherence to support plans.

Record Keeping.

Maintain precise and current documentation of individual progress, support plans, and any significant changes, while upholding legal and ethical confidentiality standards.

Family Education & Support.

Offer education and assistance to families, helping them navigate the service system and equipping them with essential tools and strategies to effectively support their loved ones.

Ongoing Professional Development.

Stay informed about the latest advancements in the field, including new services, interventions, and resources, through continuous training and education to ensure the provision of high-quality, evidence-based care.

Qualifications:

  • A bachelor's degree with 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or related social sciences; or
  • A combination of 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or related social sciences and two years of professional experience in developmental disabilities, special education, mental health, counseling psychology, social work, or health and rehabilitative services.

Requirements for Employment:

Valid automobile and insurance, valid driver's license, acceptable motor vehicle record, clearances as required, social security card, immigration documentation, and drug screening clearance.

Work Environment:

Prolonged periods of computer work in an open office setting, comfortable with virtual communication, and potential travel within the assigned region, including adverse weather conditions. Ability to lift up to 15 pounds may be required.
