About the Role

We are seeking a highly skilled Care Coordinator to join our team at Children's Health. As a Care Coordinator, you will play a critical role in coordinating care throughout the continuum of care for an assigned patient population.

Key Responsibilities
  • Utilize advanced nursing skills and knowledge to coordinate care and facilitate transitions between healthcare settings.
  • Work collaboratively with the multidisciplinary team, including providers, nurses, social workers, and other care team members to ensure seamless care coordination.
  • Advocate for the needs of patients and families, and work to identify and implement initiatives to improve processes and outcomes.
  • Develop and maintain effective relationships with patients, families, and healthcare providers to ensure high-quality care and patient satisfaction.
  • Stay up-to-date with industry developments and best practices in care coordination and transition management.
Requirements
  • At least 4 years of experience in pediatric nursing, inpatient case management, care management, care coordination, utilization review, or community-based nursing.
  • BSN required.
  • Registered Nurse in the State of Texas upon hire.
  • Accredited Case Manager (ACM) or Certified Case Manager (CCM) or Care Coordination and Transition Management (CCTM) certification preferred.
