About the Role:
The RN Care Coordinator plays a crucial role in managing discharge planning from the moment of patient admission, continuously evaluating and adjusting the plan throughout the hospital stay.
Key Responsibilities:
- Lead a multidisciplinary team in developing, refining, and communicating care plans for a defined patient population.
- Implement clinical pathways, addressing the educational needs of patients and families while monitoring for any deviations from the plan.
- Collaborate with community liaisons to ensure seamless transitions between care settings and maintain communication with ongoing providers.
- Work alongside Social Work, Utilization Review, Clinical Documentation Specialists, and others to optimize financial and care resources for patients and families.
- Coordinate with the care team to enhance patient throughput and reduce length of stay.
- Conduct daily rounds on patients to ensure effective preparation for discharge, confirming that all learning and discharge objectives are met.
- Communicate effectively with healthcare providers, patients, families, and hospital personnel, demonstrating proficiency in interactions with diverse age groups.
- Act as a liaison with various departments to ensure timely receipt of pending orders and results.
- Ensure follow-up appointments are scheduled prior to patient discharge.
- Document discharge instructions related to care coordination needs, ensuring understanding by patients and their families.
- Collaborate with physicians, unit managers, and interdisciplinary teams to facilitate safe and timely discharges.
Qualifications:
- Graduate of an accredited nursing program.
- A minimum of 2 years of experience as a Registered Nurse is required.
- Current Texas (or NLC Nursing Licensure Compact) license.
- Current Basic Life Support (BLS) certification from an accredited provider.
- Experience in Care Coordination or Case Management within an acute care setting is highly preferred.
- Proficiency in reading, writing, and communicating in English is essential.
