Hospice Nurse

At Falcon Healthcare, we are seeking a compassionate and skilled Hospice Nurse to join our team. As a Hospice Nurse, you will play a vital role in providing high-quality patient care and services to in-home clients.
Key Responsibilities:- Assure quality patient care and services are provided to in-home clients
- Make decisions based on educational preparation and experience in nursing
- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with interdisciplinary teams to ensure comprehensive care
- Provide emotional support and education to patients and their families
- Current RN License – active and in good standing
- Minimum of 1-year nursing experience preferred
- Hospice experience is a plus
- Current CPR certification
- Strong organizational and communication skills
- Proof of current auto insurance
- Proof of Covid vaccine, medical exemption or religious exemption
