We are seeking a skilled Hospice Nurse Practitioner to join our team at CURO Health Services. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and support to individuals and their families.
Key Responsibilities- Conduct face-to-face encounters with patients to gather clinical information and support hospice diagnosis and prognosis.
- Communicate findings to the hospice physician responsible for certifying or recertifying patient eligibility.
- Perform physical assessments and provide clinical findings to inform patient care.
- Collaborate with the interdisciplinary team to develop and implement patient care plans.
- Master's or higher degree in Nursing.
- Completion of a graduate-level nurse practitioner education program accredited by a national accrediting body.
- Current state license as a registered nurse and current national certification as a Nurse Practitioner (ANCC or AANP).
- Collaborative agreement in place.
