Job Summary:The Hospice CNA will provide high-quality care to patients in their homes, hospitals, or long-term care facilities. This role requires a strong understanding of hospice care principles, excellent communication skills, and the ability to work collaboratively with our interdisciplinary team.
Responsibilities:- Administer pain management medications and perform prescribed treatments
- Monitor and report changes in patients' conditions to physicians and other healthcare professionals
- Maintain accurate and up-to-date medical charts, patient files, and health records
- High School Diploma or equivalent
- Current driver's license and valid auto liability insurance
- At least one year of experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice, or long-term care facility
- Successful completion of the Aide Competency Evaluation Skills Checklist and written Skills Test
