Hospice Care RN Job Description

We are seeking a compassionate and skilled Hospice Care RN to join our team at The West Texas Rehabilitation Center.

Job Summary

The Hospice Care RN will provide skilled nursing care to patients and their families in response to crisis calls and participate in interdisciplinary care. The ideal candidate will have excellent listening and communication skills, with the ability to triage calls and provide home or in-patient visits as needed.

Key Responsibilities
  • Triage patient calls and provide home or in-patient visits for unexpected changes in condition, deaths, disposal of narcotics, and when patients need warrant.
  • Supervise and give reports to weekend LVN/Aide.
  • Contact secondary on-call RN to report patient-related calls when a backup is needed, for all referrals received, when other disciplines are needed, and matters unrelated to patient care.
  • Contact Administrator on-call for status changes when secondary RN cannot be reached.
  • Facilitate hospice transfer to in-patient setting, notifying hospital or skilled nursing facility admissions department/supervisor and/or emergency room of patients identity.
  • Attempt to contact the patients assigned volunteer when the patient is actively dying.
  • Notify team members of all deaths on voicemail.
  • Assess client outcomes.
  • Attend death, pronounce time of death, provide emotional support, provide post-mortem care and do patient discharge.
  • Answer patient/family questions and concerns relative to hospice care and services.
  • Teach, supervise, and counsel patient/family/caregivers regarding physical care, symptom management, and pain control.
  • Actively participate with the IDT in the development of care plan as appropriate.
  • Attend IDT.
  • Document clearly and concisely in a timely manner.
  • Receive report at initiation of shift and report shift interventions to backup RN by 8:00 AM.
  • Practice only within the designated Scope of Practice as defined by the Texas State Board.
  • Recognize and report situations of potential harm to the patient.
  • Participate in continuing education opportunities and in-services.
  • Provide a safe and comfortable environment.
  • Maintain the patients and familys right to privacy and confidentiality at all times.
  • Work non-judgmentally with patients and families of various cultures, religious, and socioeconomic backgrounds and lifestyles.
  • Deal effectively with difficult situations by using negotiation skills to resolve conflict.
  • Exhibit good listening and comprehension.
  • Treat patients, families, and staff with respect and compassion.
  • No gossip, does not harm others in conversation.
  • Perform other responsibilities as assigned.

This position requires endurance to perform tasks over long periods of work hours and the ability to perform essential functions without posing a direct threat in the workplace.

Must be able to lift 55 pounds.
