Medical Care Coordinator

STARS Adult Medical Day Care Cntr L seeks a highly skilled Registered Nurse to provide exceptional patient care. This role involves coordinating with colleagues to assess, plan, implement, and evaluate patient care plans.
Responsibilities- Patient Care Coordination: Work closely with healthcare professionals to create personalized care plans for patients.
- Care Planning Conferences: Conduct quarterly care planning conferences for each participant enrolled.
- Nursing Services: Provide nursing care based on physician's orders and daily assessment of each participant's health status.
- Staff Education and Consultation: Participate in staff education activities and provide consultation to facility personnel.
- Record-Keeping: Maintain accurate records of patient care and discuss observations with the supervisor.
- Licensure: Currently licensed Registered Nurse (RN) in state of employment.
- Experience: Previous experience as a Registered Nurse is preferred.
- Skills: Excellent interpersonal and client care skills.
- Certifications: Valid driver's license and First aid/CPR certification required.
The estimated annual salary for this position is $65,000-$80,000, depending on qualifications and location.
