">We are seeking a highly skilled Registered Nurse to join our team as a Care Coordinator for adults with complex needs. As a member of our team, you will be responsible for coordinating patient care, working closely with healthcare professionals to ensure seamless transitions between settings, and advocating for patients' needs.
">Key Responsibilities:
">Coordinate patient care plans with interdisciplinary teams to ensure comprehensive and coordinated care.
Develop and maintain strong relationships with patients, families, and healthcare providers to ensure optimal patient outcomes.
Collaborate with BAYADA staff to ensure timely and effective communication about patient status, concerns, and goals.
Analyze patient data and identify areas for improvement to optimize care delivery.
Requirements:
">To be successful in this role, you will need:
">A current RN license in the state where services are provided.
At least 1 year of experience in home health care or a related field.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a team.
Basic computer skills and proficiency in electronic medical records (EMRs).
