**Job Summary:**
We are seeking an experienced Patient Navigator to join our team. The ideal candidate will have a strong background in healthcare, excellent interpersonal skills, and the ability to work well under pressure. In this role, you will be responsible for coordinating patient care, navigating complex healthcare systems, and providing individual support to ensure timely appointments and follow-up care.

**Key Responsibilities:*** Bridge the gap between clinicians and patients by providing individual support and assistance in navigating the healthcare system
* Collaborate with RN Navigators, Social Workers, and other healthcare professionals to provide comprehensive care
* Conduct daily multidisciplinary rounding on the inpatient oncology unit to identify needs and opportunities for coordination of care, follow-up, and scheduling
* Act as a member of the Transition of Care team to assist patients in transitioning from inpatient to outpatient areas without interruption in services and care
* Assist patients in connecting with services for support, such as social work and financial counseling

**Requirements:**
* 1-2 years of experience working in a healthcare office setting or community outreach and education required
* Strong knowledge of healthcare, inpatient/outpatient settings, and oncology-specific patient population preferred
* Excellent interpersonal skills, ability to work well with people of various backgrounds, ethnicities, and life experiences
* Ability to take direction, follow through with tasks, and work independently
* Flexibility, optimism, and a cooperative working style required

**Salary Estimate:** $65,000 - $80,000 per year (based on location and experience)
