Care Coordination Specialist

We are seeking a skilled Care Coordination Specialist to join our team at Cooper University Hospital.
The successful candidate will be responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating healthcare options and services to enhance the health status of identified populations.
The role involves following established workflow for utilization and self-management support, participating in PCMH activities, collaborating with patients' medical/health and community-based providers, providing outreach, care management, and education for disease self-management, and identifying, documenting, and mitigating patient barriers to improved outcomes.
Requirements- 5+ years of clinical experience, preferably in primary care, population or case management.
- RN (BSN) preferred.
- Current NJ RN License.
- Strong analytical, interpersonal, communication, and time management skills.
- Knowledge of disease management, expected treatments, and discharge planning.
- Commitment to patient advocacy, with the ability to work independently and make sound clinical decisions.
