Job Overview

This role involves providing critical support to patients and families navigating complex healthcare systems. You'll use your expertise to facilitate communication between healthcare professionals, insurers, and community-based programs.

Key Responsibilities
  1. Conduct thorough needs assessments to develop personalized care plans
  2. Collaborate with interdisciplinary teams to coordinate care transitions and ensure continuity of care
  3. Negotiate with insurance providers and utilization management departments to secure necessary resources
Qualifications

To excel in this role, you'll require 3-5 years of healthcare experience and a graduate degree in social work or nursing. Current licensure as a NJ SW or RN is mandatory. Certification in case management is a plus.

Compensation Package

We offer an attractive compensation package, including an estimated annual salary of $90,000 - $115,000, depending on your background and experience.
