Job Summary

Alta California Regional Center (ACRC) is seeking a highly skilled Developmental Disability Case Manager to provide exceptional case management and advocacy services to individuals with developmental disabilities. This is a full-time position located in the Sacramento office, serving children with developmental disabilities ages 3-5.

Key Responsibilities:
  • Participate in the development of the Individual Program Plan (IPP) and monitor its implementation.
  • Assist clients and their families in acquiring and maintaining assessed supports and services.
  • Serve as an advocate for the client with community agencies, ensuring seamless coordination of services.
  • Consult with and assist vendors with certifications, applications, and referrals.
  • Complete all required forms, documentation, and reports in a timely and accurate manner.
  • Rotate officer-of-the-day duties with other staff members.
  • Assist colleagues with special projects, unique problems, vacation, and sick relief as needed.
  • Provide emergency on-call services to assigned clients when required.
Requirements:
  • Bachelor's or Master's degree in Social Work or a related field.
  • Excellent written, verbal, and interpersonal communication skills.
  • Strong time management and organization skills.
  • Valid California driver's license and automobile liability insurance.
  • One year of professional experience in social work or a related field, preferably in developmental disabilities.
