Role Summary

The Senior Case Management Specialist plays a crucial role in delivering case management services aimed at assisting clients in securing income, insurance, and stable housing. This position addresses co-occurring disorders, enhancing clients' functioning and skill development through assessment, engagement, and access to necessary treatments.

Qualifications Required

  • High School Diploma/GED (SUD Certification or AA/Bachelor's degree preferred) with a minimum of 12 months of relevant case management experience.
  • Ability to represent WellSpace Health positively and professionally.
  • Collaborative team player who values diverse perspectives.
  • Experience in Harm Reduction, Motivational Interviewing, and Cultural Humility, particularly concerning Substance Abuse.
  • Understanding of appropriate professional boundaries with clients and colleagues.
  • Familiarity with community resources and housing application processes.
  • Capacity to thrive in a dynamic work environment.
  • Proficient in documenting progress notes and maintaining comprehensive records.
  • Commitment to serving underserved populations.
  • Willingness to participate in after-hours on-call duties as necessary.
  • Excellent verbal and written communication skills.
  • Proficient in Microsoft Office Suite and database management.
  • Current CPR and first aid certification or willingness to obtain within the first 90 days of employment.
  • Valid driver's license and personal vehicle for patient transportation.
  • Adherence to professional ethics and HIPAA confidentiality standards.
  • Compliance with infection control protocols.
  • Support for the Center/Department/Program in achieving High Reliability standards.

Key Responsibilities

  • Conduct individual intake assessments for each client.
  • Advocate for clients in securing identification, housing, and benefits from various agencies.
  • Assist clients in completing housing applications and plans.
  • Support clients in maintaining stable housing.
  • Engage in care coordination meetings with relevant partners.
  • Facilitate connections to community resources and support programs.
  • Collaborate with clients to develop personalized SMART treatment plans.
  • Document daily progress notes in electronic health record systems.
  • Participate in ongoing training and certification renewal.
  • Attend regular team and management meetings.
  • Connect clients with medical insurance and health home services.
  • Assist clients in scheduling healthcare appointments.
  • Provide transportation support for clients to access resources and appointments.
  • Maintain accurate records for reporting purposes.
  • Adapt to varying work locations based on program needs.
  • Perform additional duties as assigned.
