Job Summary

We are seeking a highly organized and detail-oriented Referral Coordinator to join our team at CommuniCare Health Center. As a Referral Coordinator, you will play a critical role in ensuring seamless communication between patients, providers, and staff.

Key Responsibilities
  • Schedule patient appointments efficiently and accurately, utilizing established provider templates.
  • Verify patient demographics and insurance information in the Electronic Health Record (EHR) system, ensuring completeness and accuracy.
  • Process referrals according to the oldest date ordered and level of urgency, tracking and managing referrals throughout every stage to avoid backlog and ensure completion.
  • Provide excellent internal and external customer service via telephone, messaging system, and fax contact to assist patients with their healthcare needs.
  • Perform other related duties as assigned.
Requirements
  • High School graduate or equivalent required.
  • Minimum of one year of experience in customer service, preferably in a healthcare setting.
  • Experience with EMR/EHR and medical terminology preferred.
  • Excellent oral and written communication and interpersonal skills.
  • Ability to handle multiple tasks efficiently with minimal supervision.
Working Conditions

This position requires the ability to sit for long periods, with occasional bending, stooping, and standing. The ability to lift folders, files, papers, equipment, and other items weighing up to approximately 25 lbs is also required.
