We are the largest long-term care facility in South Dakota, offering a range of services including general long-term care, short-stay rehabilitation, and specialized memory care.

Role Summary


The Senior Nursing Assistant (NA) acts as a skilled caregiver to residents during their scheduled shifts in long-term care settings.

This role requires advanced expertise as a certified nursing assistant and the ability to support other staff as necessary.

Responsibilities include delivering resident-focused nursing care and daily living assistance to assigned individuals under the supervision of a registered nurse (RN).

Possesses knowledge of and provides age-appropriate care tailored to the physical and psychological needs of residents as outlined in their care plans.

As a vital member of the nursing team, accountability for adherence to infection prevention protocols and the proper use of Personal Protective Equipment is essential.

Frequently provides support to other clinical staff while managing personal responsibilities. Takes the lead in problem-solving and maintaining high standards of accountability.


Assists with fundamental health care needs, including daily living activities such as bathing, grooming, dressing, obtaining and recording vital signs, and offering psychosocial support and personal care to assigned residents.

Facilitates resident mobility through safe transfer techniques and equipment, while also providing range of motion and passive exercises.

Responsible for tasks related to meal preparation and dining, ensuring a positive dining experience for residents. Aids residents with meals and snacks, providing hydration and nutritional supplements as appropriate.

The NA assists healthcare providers and nursing staff with procedures as needed. Responsible for documenting resident interactions and outcomes related to care, behavior, activities, and dietary needs. Communicates any changes in residents' conditions and care-related concerns to the charge nurse. Collects non-invasive body fluid specimens and gathers vital signs. Provides housekeeping and laundry services to maintain clean and functional living spaces. Supports wellness and recreational programs, encouraging resident participation. May take part in departmental committees and task forces as required.

May also be tasked with responsibilities related to the Restorative Nursing program, including documentation, scheduling, and leading restorative activities.

Qualifications
High school diploma or equivalent preferred, but not mandatory.

Minimum age of 16. Two years of clinical or nursing assistant experience in long-term care preferred.

Required Certified Nursing Assistant (CNA) certification with the State Board of Nursing.

Must complete annual department-specific competency testing and maintain a record of continuing education credits each year, with a minimum of 12 hours required annually. All department-specific certifications must be maintained.

Completion of Restorative Nursing training may be required within six months.

For applicable states, certification and registration with the Unlicensed Assistive Person (UAP) Registry may be necessary.

Depending on facility requirements, this role may classify as a Sanford Category II Driver, necessitating a motor vehicle report and proof of a valid driver's license. Must also meet medical qualifications to perform essential driving functions as per organizational policy.
