Job Summary

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at IntelyCare. As an LPN, you will provide high-quality patient care in post-acute facilities, working closely with our team to ensure the best possible outcomes for our residents.

Responsibilities
  • Provide compassionate nursing care to residents in post-acute facilities
  • Monitor residents' physical and mental conditions, tracking vital signs and documenting medical histories
  • Administer and verify medications, tracking their use and effects
  • Cultivate a comfortable and safe facility environment for residents
  • Track residents' medical records and charts
  • Assist with IV setup, immunizations, and phlebotomy procedures
  • Maintain accurate medical supply inventory
  • Adhere to resident care plans and follow established protocols
  • Participate in staff orientation and training of new colleagues
Requirements
  • Current New Jersey state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of an approved Licensed Practical Nurse (LPN) educational program
  • High school graduation, GED, or equivalent
  • At least 6 months of experience as an LPN or in a similar role
  • Ability to lift a minimum of 50 lbs
