Role Summary

We are currently seeking a dedicated Physical Therapist to join our esteemed team at Carnegie Post Acute Skilled Nursing at Princeton. This role is pivotal in enhancing the quality of life for our residents through tailored therapeutic interventions.

Key Responsibilities
  • Collaborate effectively with the rehabilitation team and other healthcare professionals.
  • Conduct comprehensive evaluations and maintain accurate documentation.
  • Provide insightful recommendations that support patient assessments and communications.
  • Deliver therapy services in accordance with established treatment plans and medical prescriptions.
  • Implement treatment strategies aimed at addressing patients' physical challenges and enhancing their functional abilities.
  • Manage individual and group treatment workflows, ensuring timely documentation in electronic health records.
  • Demonstrate familiarity with the Patient-Driven Payment Model (PDPM).
  • Align therapy practices with Medicaid and Medicare billing requirements.
Qualifications
  • Valid New Jersey Physical Therapy License.
  • Experience in long-term care settings, particularly within skilled nursing facilities.
  • Exceptional organizational, interpersonal, and communication skills.
  • Ability to engage with patients of diverse personalities and varying ability levels.
  • Strong problem-solving capabilities.
