Position Overview:


The Clinical Dietitian will play a crucial role in offering nutritional counseling and support to patients at JWCH, Wesley Health Centers.

This position entails creating and executing tailored nutrition care plans, performing nutritional evaluations, and educating patients on healthy dietary practices to foster overall health and wellness.

The role requires collaboration with healthcare professionals to incorporate nutrition into patient care, contributing to JWCH's mission of enhancing the health status and well-being of underserved populations in the Los Angeles area.


Key Responsibilities:
  • Conduct thorough nutritional evaluations for patients, including dietary histories and lifestyle assessments.
  • Contribute to the development of nutrition programs by identifying the dietary needs of at-risk individuals, outlining issues that can be addressed through proper nutrition.
  • Monitor patient progress and modify nutrition plans as necessary to ensure optimal health outcomes.
  • Provide individualized nutrition counseling and education on topics such as weight management, chronic disease management, and overall wellness.
  • Facilitate group nutrition education sessions and workshops for patients and community members.
  • Collaborate closely with physicians, nurses, and other healthcare professionals to integrate nutrition care into comprehensive patient treatment plans.
  • Effectively communicate with patients and their families to offer support and guidance on nutrition-related matters.
  • Develop and implement nutrition programs and initiatives to meet community health needs.
  • Assess the effectiveness of nutrition programs and services, making improvements as necessary.
  • Maintain accurate and current patient records, including progress notes and care plans.
  • Ensure adherence to clinic policies, procedures, and regulatory standards related to nutrition and dietetics.
  • Stay informed about the latest research, trends, and best practices in nutrition and dietetics.
  • Participate as a member of Continuous Quality Improvement (CQI) and other assigned committees.
  • Perform additional tasks as assigned.

Qualifications:
  • Bachelor's degree in Dietetics, Nutrition, or a related field from an accredited institution.
  • Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) Certification is required.
  • A minimum of two years of experience in a relevant dietary field.
  • Experience working with diverse patient populations and managing chronic health conditions.
  • Strong understanding of nutrition science, dietary guidelines, and evidence-based practices.
  • Excellent interpersonal and communication skills, with the ability to educate and motivate patients effectively.
  • Ability to work collaboratively within a multidisciplinary team environment.
  • Proficiency in using electronic health records (EHR) and other relevant software.
  • Strong organizational skills and attention to detail.
  • Ability to handle sensitive information with confidentiality and professionalism.
  • Current California Driver's License.
  • Ability to travel to multiple work sites.
  • Bilingual (English/Spanish) ability is preferred.
