Position Overview: The Clinical Coordinator plays a crucial role in managing and enhancing patient care services. Under the guidance of a Registered Nurse Clinical Supervisor, this position collaborates with and supports the clinical field team.

Key Responsibilities:

  • Coordinate multiple facets of patient care, including physician orders, patient assessments, and the review and approval of supply requests.
  • Manage admission logs and follow up on lab assignments as necessary.
  • Ensure high standards of care are maintained, facilitating a seamless and supportive experience for patients and their families throughout their home healthcare journey.

Requirements:
  • Current and unrestricted CA LVN License.
  • Minimum of 1 year of LVN experience in a direct patient care environment.
  • Current AHA CPR certification.
  • Valid Driver's License.

Salary:
$40-50/hr
