About the Role

We are seeking a highly skilled and compassionate Lead Care Manager to join our Enhanced Care Management (ECM) program at Seneca Family of Agencies. As a key member of our team, you will play a vital role in providing comprehensive care management and support to high-need youth and families.

Key Responsibilities
  • Conduct outreach and engagement with new clients to establish rapport and trust
  • Collaborate with youth, families, and care teams to assess needs and develop comprehensive care plans
  • Coordinate care activities and maintain communication with external providers, including county substance use disorder and specialty mental health providers as appropriate
  • Manage referrals, coordination, and follow-up to needed services and supports
  • Provide education and identify support needs for youth and their families
  • Build and foster collaborative relationships with community resourcing agencies
  • Provide services in-person and accompany clients to appointments when needed
  • Complete required documentation in a timely manner
  • Attend supervision and other team meetings as required
Requirements
  • Bachelor's degree in Social Work, Psychology, or Counseling, plus 1 year of experience, or equivalent combination of education and experience
  • Experience working with families who are experiencing the impact of complex systematic disparities that contribute to community and individual mental health challenges
  • Flexibility to work some earlier mornings or evenings, scheduled ahead of time
  • Must be at least 21 years of age
  • Own vehicle, valid California driver's license, and insurability with Seneca's automobile policy
  • TB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirements
  • Must show proof of COVID-19 vaccination
Preferred Qualifications
  • Prior case management and community-based experience
  • Bilingual Spanish skill set
  • Strong organizational skills as demonstrated by the ability to effectively manage multiple tasks
Work Schedule
  • Full-time
  • Monday-Friday, 9am-5pm
  • Flexible schedule with ability to work earlier mornings or evenings as needed
  • Combination of in-person and remote work

  • Starting range: $25.84 per hour
  • Actual salary dependent on creditable experience above the minimum qualification
