Key Responsibilities- Conduct outreach and engagement with new clients to establish rapport and trust
- Collaborate with youth, families, and care teams to assess needs and develop comprehensive care plans
- Coordinate care activities and maintain communication with external providers, including county substance use disorder and specialty mental health providers as appropriate
- Manage referrals, coordination, and follow-up to needed services and supports
- Provide education and identify support needs for youth and their families
- Build and foster collaborative relationships with community resourcing agencies
- Provide services in-person and accompany clients to appointments when needed
- Complete required documentation in a timely manner
- Attend supervision and other team meetings as required
- Bachelor's degree in Social Work, Psychology, or Counseling, plus 1 year of experience, or equivalent combination of education and experience
- Experience working with families who are experiencing the impact of complex systematic disparities that contribute to community and individual mental health challenges
- Flexibility to work some earlier mornings or evenings, scheduled ahead of time
- Must be at least 21 years of age
- Own vehicle, valid California driver's license, and insurability with Seneca's automobile policy
- TB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirements
- Must show proof of COVID-19 vaccination
- Prior case management and community-based experience
- Bilingual Spanish skill set
- Strong organizational skills as demonstrated by the ability to effectively manage multiple tasks
- Full-time
- Monday-Friday, 9am-5pm
- Flexible schedule with ability to work earlier mornings or evenings as needed
- Combination of in-person and remote work
- Starting range: $25.84 per hour
- Actual salary dependent on creditable experience above the minimum qualification
- Additional compensation provided upon passing bilingual language proficiency exam
- Mileage reimbursement
- Salary increases each year
