Job Description

SUMMARY

The Healthcare Quality Coordinator plays a vital role within the Value-Based Care/Quality Team. This position is designed to support patients and healthcare providers within the Louisiana State University School of Medicine. The integrated model aims to improve patient outcomes while effectively managing healthcare expenses. Rather than providing direct patient care, the Healthcare Quality Coordinator acts as a crucial link between patients, caregivers, providers, and the Value-Based Care team.

Business Hours: Monday through Friday

**No nights, weekends, or major holidays**

Key Responsibilities

  • Guide and assist patients throughout their entire healthcare journey.
  • Schedule various patient appointments, including routine visits, wellness checks, and lab tests.
  • Contact patients to address specific healthcare metrics, aiming to close care gaps and enhance quality scores.
  • Collaborate with the Director of Quality to develop and implement effective care management strategies.
  • Conduct manual audits of patient records across different Electronic Medical Records (EMRs) to ensure compliance.
  • Work with the Director of Quality and billing teams to resolve coding and claims issues.
  • Travel to multiple clinic locations as necessary.
  • Follow up with patients regarding health checks, lab results, and screenings.
  • Serve as the primary contact for patients and caregivers.
  • Prepare for patient visits by documenting care reminders and notes for healthcare providers.
  • Communicate directly with healthcare providers as needed.
  • Assist patients with various needs, including medication refills and referrals.

Qualifications:

  • High school diploma or equivalent required.
  • Minimum of 1 year of clinical experience required.
  • Medical Assistant Certification preferred.
