Community Support Specialist

The Thresholds Homeless Outreach Program (HOP) has been a vital service since 1989, dedicated to providing proactive outreach, connection to essential health services, and resource guidance for individuals facing homelessness and serious mental health challenges. Our program uniquely operates throughout the city, ensuring that we reach those in need wherever they may be.
ROLE SUMMARY
The Outreach Worker will collaborate with a dedicated team to engage and support individuals experiencing homelessness who are dealing with significant mental health issues. The primary focus will be on assessing needs and facilitating access to housing, benefits, treatment, advocacy, and other necessary services. The Outreach Worker will also play a key role in creating and executing a personalized service plan in partnership with clients and the outreach team to achieve set objectives.
Key Qualifications:
- Strong communication and interpersonal skills
- Proficient writing abilities
- Excellent organizational and time management skills
- Minimum age of 21 years
- Possession of a valid driver's license and current vehicle insurance, with daily access to a personal vehicle preferred
- Willingness to provide transportation for clients
Educational and Experience Requirements:
- High school diploma or GED with 5 years of supervised clinical experience
- Bachelor's degree in Psychology, Social Work, or a related discipline
- Bachelor's degree in a different field with 2 years of supervised clinical experience
- Master's degree in Psychology, Social Work, or a related area
