Position Overview

The Thresholds Homeless Outreach Program (HOP) has been dedicated to providing essential outreach services since its inception in 1989. Our mission is to connect individuals facing homelessness, particularly those with significant mental health challenges, to vital resources and support systems. As the only citywide mental health outreach initiative in Chicago, we prioritize proactive engagement, ensuring that our services reach those in need directly.

Role Summary

The Community Support Specialist will collaborate with a dedicated team to identify and engage individuals experiencing homelessness who are dealing with serious mental health issues. The primary focus will be on assessing their needs and facilitating access to housing, benefits, treatment, and advocacy services. This role also involves creating and executing a personalized service plan in partnership with clients and the outreach team to achieve their specific objectives.

Key Qualifications:

  • Strong interpersonal and communication skills
  • Proficient writing abilities
  • Excellent organizational and time management skills
  • Minimum age of 21 years
  • Possession of a valid driver's license and current car insurance, with daily access to a vehicle preferred
  • Willingness to provide transportation assistance to clients

Educational and Experience Requirements:

  • High school diploma or GED with 5 years of supervised clinical experience
  • Bachelor's degree in Psychology, Social Work, or a related discipline
  • Bachelor's degree in a different field accompanied by 2 years of supervised clinical experience
  • Master's degree in Psychology, Social Work, or a related area
