Job Overview

Under guidance, this role primarily involves engaging with communities to provide crisis intervention and social services support for individuals facing mental health challenges and substance use issues. The position entails various responsibilities, including:

Key Responsibilities:
  • Responding to referrals from emergency services for individuals with behavioral health needs.
  • Conducting comprehensive assessments to gather essential information and establish both short-term and long-term case management objectives for clients.
  • Following up post-crisis to ensure clients receive necessary support and resources.
  • Offering both in-person and telephonic case coordination for clients dealing with substance use and mental health crises.
  • Assisting clients in attending appointments, including transportation as needed.
  • Collaborating with first responders during referrals or dispatches.
  • Working with public safety agencies to manage cases of individuals frequently interacting with law enforcement or emergency services.
  • Coordinating with clients, families, and field team members to assess service levels and needs.
  • Engaging with emergency response agencies to evaluate if clients require higher levels of care.
  • Participating in community outreach events to raise awareness about available mental health services.
  • Building networks with social service organizations to create referral pathways for clients.
  • Staying informed about eligibility criteria for various social service programs.
  • Assisting clients in applying for federal and state benefits.
  • Maintaining accurate client records using designated case management systems.
  • Advocating for clients with external agencies and service providers to meet ongoing needs.
  • Establishing strong relationships with city departments, healthcare professionals, and community organizations to deliver services to high-risk clients.
  • Traveling to various locations to engage with unsheltered individuals in a compassionate and trauma-informed manner.
  • Providing support during the activation of emergency shelters.
  • Collaborating with outreach partners to connect clients with essential services.
Additional responsibilities may be assigned as necessary.

Qualifications

MINIMUM REQUIREMENTS:
  • Bachelor's degree in Social Sciences or a related field from an accredited institution; AND
  • Two years of experience in community or social services, particularly with unsheltered populations or individuals in crisis; OR
  • An equivalent combination of education, training, and experience.
Licensure and Certification
  • A valid driver's license is required.
  • Must maintain valid car insurance.
  • Flexibility to work various hours as needed.
  • Preference given to candidates with experience in Assertive Community Treatment or Community Support Teams.
WORKING CONDITIONS
  • Exposure to outdoor elements.
  • Potential hazards in navigating certain environments.
  • General office environment.
PHYSICAL REQUIREMENTS
  • Ability to navigate staircases.
  • Capacity to walk and stand for extended periods.
KNOWLEDGE AND SKILLS
  • Understanding of social service programs and resources.
  • Familiarity with case management techniques.
  • Aware of the specific needs and challenges faced by various communities.
  • Knowledge of relevant laws and regulations.
