Job Summary

We are seeking an experienced and dynamic Home Health Sales Clinical Community Liaison / Care Navigator to join our team at Jet Health. The primary responsibility of this role is to drive sales and develop relationships with key accounts, generating referrals and growing the agency's client base for our hospice office.

Key Responsibilities
  • Articulate Service Capabilities: Effectively communicate the value proposition of our home health services to targeted audiences.
  • Coordinate Referrals: Collaborate with the intake/office team to ensure timely referrals and communication.
  • Proactive Lead Follow-up: Regularly follow up on new account leads to drive sales growth.
  • Community Needs Assessment: Provide counsel to the agency regarding the needs of the community, including program development and personnel requirements.
  • Policy Development: Collaborate with administrative and supervisory personnel to develop and maintain policies specific to new programs.
  • Compliance: Ensure compliance with all local, state, and federal laws relating to marketing of the agency.
  • Program Direction: Assist in the direction of programs through collaboration and delegation of responsibility.
  • Public Relations: Establish a public relations program to interpret the agency's services and foster good working relations with physicians and community agencies.
  • Relationship Building: Build and nurture viable, value-based relationships within key accounts.
  • Referral Generation: Generate referrals from physicians, discharge planners, social workers, case managers, SNFs, and other healthcare settings/professionals.
  • Sales Planning: Develop a workable sales plan, identifying key account targets and associated tactics to execute each week.
  • Market Intelligence: Utilize market intel data and available tools to aid in growth.
  • Community Engagement: Attend community, networking, and social events and gatherings.
Requirements
  • Education: Graduate of an accredited college/university required as a LPN / LVN, RN, Registered Nurse, Therapist (PT, OT, SLP) or Social Worker (BSW, MSW, LBSW, LMSW, LCSW, LICSW, LMFT, LPC).
  • Experience: Previous post-acute healthcare sales experience preferred, especially in SNFs, DME, ortho, and cardiac. Home Health sales highly preferred.
  • Skills: Excellent communication, selling, and problem-solving skills. Ability to articulate service capabilities to the targeted audience.
  • Personal Qualities: Go-getter attitude, outgoing and driven.
  • Transportation: Reliable transportation.
