Position Overview: The role entails overseeing and managing client services programs, ensuring proper documentation and follow-up procedures are adhered to.

Key Responsibilities:

1. Evaluate all clients for program eligibility while assessing patient needs through direct interviews.

2. Conduct in-person evaluations and re-evaluations biannually.

3. Confirm that all clients referred to services meet eligibility criteria prior to service initiation.

4. Investigate and validate clients' insurance coverage.

5. Provide support with referrals and coordinate services in alignment with agreements established with other organizations, as well as federal and local programs.

6. Maintain the accuracy of eligibility records through a systematic audit review process.

7. Diligently document all client interactions within software systems, complete and submit billing documentation, and manage physical client files.

8. Adhere to all program requirements and stay informed of any changes in policies.

9. Collaborate with clinic team members to ensure seamless coordination of patient services.

10. Process necessary documentation as mandated by state and local funding sources.

11. Uphold confidentiality regarding patient and family information in accordance with professional standards.

Additional Responsibilities:

1. Cross-train in prevention, outreach, and testing initiatives for effective team engagement.

2. Build and nurture networking relationships with community and support resources.

3. Implement safety, environmental, and infection control practices.

4. Promote and embody the values and mission of CAN Community Health, adhering to all policies and procedures.

5. Engage in ongoing professional development to enhance knowledge and skills related to HIV/AIDS.

6. Participate in continuous quality improvement efforts for services provided to clients.

7. Utilize resources and supplies efficiently and responsibly.

8. Exhibit courtesy and respect in all interactions with clients, staff, and visitors.

9. Maintain a professional demeanor in all job-related activities.

10. Execute any other duties as assigned.

Reporting Structure: Reports to the Ryan White Program Coordinator.

Qualifications:

1. High School Diploma or GED required.

2. Fundamental understanding of HIV/AIDS prevention and treatment.

3. Completion of Basic Annual Safety Training, HIV 104, 500, and 501.

Physical Requirements:

1. Professional appearance is essential.

2. Sedentary role - requires exerting up to 10 lbs. of force occasionally and/or a negligible amount of force frequently or constantly to move objects, including the human body. Involves sitting most of the time but may require brief periods of walking or standing.

3. Requires effective communication through spoken word, visual, and auditory acuity.

Knowledge, Skills, and Abilities:

1. Valid driver's license is necessary.

2. Strong communication and interpersonal skills with patients, visitors, and staff.

3. Visual and auditory acuity is required.

4. Ability to apply problem-solving techniques, be self-motivated, work efficiently, stay organized, and manage multiple tasks simultaneously.

5. Capacity to read, comprehend, follow, and enforce safety protocols.

6. Demonstrated organizational skills.

7. Willingness to engage with computer software programs.

8. Ability to coordinate several tasks concurrently with accuracy.

9. Capability to work with minimal supervision.

10. Openness to travel for conferences, training, and meetings.
