Position Overview: The Clinical Supervisor for Complex Care Services plays a pivotal role in enhancing patient care by collaborating with primary care, behavioral health, and other clinical team members. This position is responsible for providing clinical guidance and oversight to a dedicated team of Case Managers and Associate Case Managers.

Working closely with the Director of Complex Care, the Clinical Supervisor ensures that patients receive top-notch case management services. Key responsibilities include:

  • Delivering direct administrative and clinical supervision to Complex Care Case Managers.
  • Offering clinical support and oversight for patient interactions, including hospital follow-ups and medication management.
  • Conducting new patient assessments alongside Associate Case Managers to maintain high-quality standards.
  • Assisting in program development, including workflow creation and grant participation.
  • Ensuring compliance with program reporting requirements and quality measures.
  • Performing home and community visits as necessary.
  • Documenting all patient interactions using the clinic's electronic health record system.
  • Participating in relevant training sessions to enhance skills in health coaching and patient engagement.
  • Providing accurate and timely data reporting for clinic programs.
  • Facilitating recruitment efforts and onboarding processes for new staff members.
  • Managing employee performance and conducting evaluations in a timely manner.

Qualifications: Candidates should possess a strong background in healthcare, with a minimum of one to two years of experience supporting individuals with chronic and complex behavioral health needs. Knowledge of state regulations and ethical standards related to patient rights is essential. Proficiency in both English and Spanish is highly preferred, particularly for conducting clinical assessments and case management interventions.
