Role Overview
The Complex Care Coordination (C3) Liaison plays a crucial role in responding to urgent referrals from warriors, conducting initial evaluations, and guiding them towards appropriate resources. This position acts as a case manager, ensuring a seamless treatment plan and addressing barriers to care. The C3 Liaison serves as an advocate, facilitating a coordinated response for warriors facing complex challenges that require comprehensive support.

Key Responsibilities
Caseload Management:
  • Serve as a subject-matter expert in mental health program planning and execution.
  • Maintain a caseload of assigned warriors, conducting thorough assessments to identify care needs.
  • Collaborate with care teams to develop and implement tailored treatment plans.
  • Foster a supportive relationship with warriors, empowering them to make informed decisions.
  • Ensure accurate documentation of services provided and maintain confidentiality of sensitive information.
Program Management:
  • Coordinate care by engaging with internal WWP programs and external organizations.
  • Manage documentation for payment facilitation and oversee invoicing processes.
  • Input and update data in relevant tracking systems to ensure accurate case management.
  • Support warriors in overcoming challenges that hinder their recovery.
Resource Development:
  • Build strong partnerships with community resources and veteran service organizations.
  • Collaborate with governmental agencies to coordinate care for warriors.
  • Prepare reports and briefings to communicate program updates.
Qualifications
Required:
  • Four years of experience in social work or a related mental health field.
  • Experience working with veteran healthcare programs and case management.
Preferred:
  • Five years of experience supporting wounded service members.
  • Master's degree in a relevant field.
Work Environment
This position is based in a general office environment, with occasional travel required. Flexibility in working hours may be necessary to meet the needs of the warriors we serve.
