Position Overview:
We are in search of a Care Coordination Specialist who will be responsible for managing care transitions for our members. This role requires residency in CALIFORNIA and the ability to travel within designated county areas.

Key Responsibilities:
The Care Coordination Specialist will engage with our Medicaid population, ensuring seamless transitions from hospital to home or other care settings. The ideal candidate will possess a minimum of one year of experience in a hospital environment, with a preference for those with discharge planning and case management experience.

Essential Duties:
- Collaborate with multidisciplinary teams to assess and coordinate care for members with complex needs.
- Facilitate safe transitions by working closely with hospital discharge planners and outpatient providers.
- Conduct face-to-face visits with members during hospital stays and follow-up home visits for high-risk individuals post-discharge.
- Utilize the Coleman Care Transitions Model to reassess member needs and coordinate care effectively.

Qualifications:
- Must be a graduate from an accredited nursing program, with a preference for a Bachelor's Degree in Nursing.
- Active and unrestricted State Registered Nursing (RN) license in good standing is required.
- A valid driver's license and a clean driving record are essential for local travel.

Skills and Competencies:
- Proficient in using electronic medical records (EMRs) and Microsoft Office applications.
- Strong attention to detail and excellent communication skills are crucial for this role.
- Ability to educate and support members in managing their health, focusing on medication management and follow-up care.

Work Environment:
- This position will involve a combination of remote work and field visits, requiring reliable transportation and a home office setup with high-speed internet connectivity.

Schedule:
- Monday through Friday, 8:30 AM to 5:30 PM PACIFIC TIME.
