Job Summary

We are seeking a highly skilled and compassionate Case Manager to join our team at Bridges Core Service Agency, Inc. As a key member of our mental health services team, you will play a vital role in providing support to consumers and their families or caregivers with mental health-related issues.

Key Responsibilities
  • Ensure Compliance: Ensure compliance with all policies and procedures, as well as local, state, and federal regulations.
  • Provide Medical Necessary Services: Provide medical necessary services to consumers in accordance with the treatment plan and diagnostic assessment.
  • Collaborate with the Service Team: Serve as an integral member of the service team to ensure continuity of care and support services 24 hours a day, seven days a week as necessary.
  • Document Encounters: Document encounters within 24-48 hours of service or phone call.
  • Participate in Supervision and Training: Report to supervision and in-house trainings as scheduled.
  • Facilitate Staff Meetings and Trainings: Facilitate or co-facilitate staff meetings and trainings.
  • Collaborate with Community Agencies: Identify, establish, and continually collaborate with community agencies, providers, and natural supports and advise the team to make referrals as needed.
  • Communicate Effectively: Listen/respond to; interact with clients, co-workers, management, and other professionals in a non-judgmental manner.
  • Participate in Quality Improvement: Participate in efforts to enhance service provision to consumers.
  • Act with Professional Work Ethics: Demonstrate professional work ethics as evidenced by overall dependability, conscientiousness, and timeliness.
  • Ensure Licenses and Certifications: Ensure all necessary licenses and/or certifications remain current.
Requirements
  • Education: Master's Degree in Social Work, Psychology, or other clinical-based discipline preferred; Bachelor's Degree from an accredited institution with at least two years of experience working in the field of mental health required.
  • Background Check: Must be able to pass a satisfactory background check, drug test, and child abuse registry.
  • Training and Certification: Ability to successfully complete service-specific training courses.
  • Computer Literacy: Proficient in clinical theories and their practical application; computer literate; experience with MS Word and Excel.
Preferred Qualities
  • Organizational Skills: Successful applicant will be well organized, have excellent time management skills, and be able to work well both independently and with a team.
  • Reliable Transportation: Have a valid driver's license, reliable transportation, and insurance.
