Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Field Case Manager. As a key member of our primary care team, you will play a vital role in coordinating care services for our members, working closely with healthcare providers, and advocating for the needs of our patients.

Key Responsibilities
  • Establish and maintain effective communication with healthcare providers, members, and caregivers to ensure seamless coordination of care.
  • Conduct thorough assessments of member needs and develop individualized care plans to achieve optimal health outcomes.
  • Collaborate with healthcare providers to ensure timely and effective delivery of care services, including medication management and disease management.
  • Provide education and support to members and caregivers on health-related topics, including disease management and wellness.
  • Participate in quality improvement initiatives to enhance the quality of care and services provided to our members.
  • Stay up-to-date on relevant healthcare policies, procedures, and best practices to ensure compliance and excellence in care coordination.
Requirements
  • Current unrestricted Registered Nurse license in Ohio.
  • 3+ years of RN experience, with a minimum of 1 year in case management or a related field.
  • Valid driver's license and ability to travel locally (up to 75% of the time).
Preferred Qualifications
  • BSN degree.
  • Experience working in post-acute care, such as long-term care or hospice, and/or case management experience as an RN.
  • Experience working with the geriatric population.
  • Basic Microsoft Office skills and ability to navigate in a Windows environment.
