Applewood Centers is seeking a highly skilled Clinical Case Manager to join our team. As a Clinical Case Manager, you will play a critical role in coordinating with community-based intervention center staff to identify youth in need of clinical case management or brief solution-focused interventions.
Key Responsibilities:- Conduct brief assessments to determine the needs of youth and develop case management services to meet those needs.
- Collaborate with CBIC staff to provide consultative support and ensure effective and safe co-facilitation of group cognitive-behavioral interventions.
- Provide support to families and program staff as needed, including travel to community and home settings.
- Work collaboratively with court administration and staff to provide consultative support and services.
- Bachelor's degree in social work or a related field, with diagnostic and clinical experience.
- Master's degree in social work or counseling, or an LSW, LPC, or MFT license is preferred.
- Prior experience in delivering mental health services in schools and communities to children and families with complex needs is essential.
