Job Summary

BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in providing high-quality care to clients in their homes. As an Outpatient Physical Therapist, you will work closely with our multidisciplinary team to develop and implement individualized care plans that promote independence and improve the overall well-being of our clients.

Responsibilities
  • Conduct thorough assessments of clients' physical abilities and develop personalized care plans to address their unique needs.
  • Provide manual therapeutic exercises, gait training, balance, and other interventions to help clients achieve their goals.
  • Collaborate with our team to ensure seamless care coordination and communication with clients, family members, and community staff.
  • Educate clients and their caregivers on proper techniques and strategies to promote safe and independent living.
  • Accurately document client progress and outcomes, utilizing our state-of-the-art electronic medical records system.
Requirements
  • A current VA Physical Therapist license.
  • Outpatient experience preferred, but not required.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills, with prior experience with electronic medical records (EMR) a plus.
