Job Title: Healthcare Risk Assessment Specialist

At i4 Search Group Healthcare, we are seeking a skilled Nurse Practitioner to fill the role of Healthcare Risk Assessment Specialist.

Job Summary:

The Healthcare Risk Assessment Specialist will be responsible for conducting health risk assessments on insured plan patients in their homes or designated areas. This role requires a strong understanding of risk assessment coding and experience working with electronic medical records.

Responsibilities:

  • Conduct health risk assessments on insured plan patients in their homes or designated areas
  • Identify active and chronic disease conditions, as well as physical, mental, and social needs
  • Obtain vital signs, measure BMI, review pharmacological therapy, and conduct physical examinations
  • Conduct necessary screenings to capture HEDIS measures and close gaps in care
  • Educate and counsel patients and families on identified conditions

Requirements:

  • Active Nurse Practitioner state license and board certifications
  • At least 1 year of experience preferred
  • Risk assessment coding experience/knowledge preferred
  • Valid state driver's license and automobile insurance
  • Strong computer skills, including Word, Excel, EMRs, and web-based applications
