Position Overview:

Responsibilities:
The Pharmacist specializing in Hematology/Oncology will be responsible for:
  • Delivering advanced clinical pharmacy services within a large healthcare facility that emphasizes teaching and clinical pharmacy practices.
  • Designing tailored therapeutic regimens based on individual patient assessments, which include determining therapeutic endpoints and appropriate medication selections.
  • Monitoring and evaluating the therapeutic and adverse effects of medications, including assessing potential drug interactions.
  • Providing recommendations to enhance drug therapy, mitigate adverse drug events, and minimize medication errors.
  • Engaging with patients through various communication methods to gather medical histories and assess for potential medication-related issues.
  • Documenting patient medication histories and providing counseling on proper medication use and adherence.
  • Serving as a drug information resource for healthcare professionals, utilizing current references to support clinical decisions.
  • Participating in the education and training of pharmacy staff, students, and other healthcare personnel.
  • Maintaining up-to-date knowledge of hematology/oncology therapeutics and pharmacy practices.
  • Contributing to the development of clinical guidelines and quality assurance initiatives.

Work Schedule:
Standard work hours are Monday to Friday, with potential for rotating holidays and variable shifts.

Travel Requirements:
Some travel may be necessary to fulfill job responsibilities.

Qualifications:
Applicants must be U.S. citizens and possess a full, unrestricted license to practice pharmacy. Candidates must also meet educational requirements from an accredited pharmacy program. Experience in a clinical pharmacy setting is preferred.
