Role Summary
The Pediatric Clinical Pharmacist Specialist in Hematology/Oncology plays a vital role in the Hem/Onc/BMT service by delivering patient care recommendations regarding pharmacotherapy, assessing drug interactions, determining appropriate drug dosages, and optimizing medication regimens to enhance patient outcomes while minimizing the risk of adverse events. This position collaborates with a multidisciplinary team to ensure effective medication therapy management for hospitalized pediatric patients with hematologic and oncologic disorders, employing evidence-based practices and participating in interdisciplinary medical rounds.
Key Responsibilities
- Conducts and documents monitoring of drug therapy and interventions related to medication dosing, administration, interactions, and adverse reactions.
- Engages actively in clinical pharmacy programs and services, providing expert consultation in specialized areas.
- Participates in daily patient care activities, clinical service meetings, and hospital committees, contributing to protocol development and quality initiatives.
- Implements established policies and procedures to guarantee safe and effective medication therapies, recommending alternatives as necessary.
- Maintains documentation of quality activities to meet established metrics and compliance with accreditation standards.
- Provides drug information and support through in-service education for pharmacy staff and clinical personnel.
- Maintains comprehensive records related to patient care activities.
- Educates pharmacy students, residents, and medical trainees on hematologic and oncologic conditions and relevant pharmacotherapy.
- Develops and participates in quality improvement projects and medication use reviews.
- Prepares data for medication utilization evaluations and other special projects as assigned.
- Demonstrates proficiency with pharmacy and clinical information systems.
- Serves as a Staff Clinical Pharmacist as needed, fulfilling essential duties for medication management.
- Contributes to the pharmacy department's cost-saving initiatives and formulary management.
- Advances the field through presentations and community education efforts.
Essential Requirements
- Bachelor's Degree in Pharmacy or Doctorate of Pharmacy.
- Current eligibility for Florida Pharmacist License, maintaining good standing throughout employment.
- American Heart Association BLS certification, kept active throughout employment.
- 4-7 years of hospital experience as a Pediatric Pharmacist or completion of a PGY-1 Pharmacy Residency in a Pediatric Setting or PGY-2 Pediatric Residency.
Knowledge, Skills, and Abilities
- Possesses specialized knowledge or completion of an ASHP-accredited residency program in pediatrics or hematology/oncology.
- Exhibits strong analytical skills to research and evaluate drug information and resolve medication-related issues.
- Understands pharmacokinetics and pharmacotherapeutic properties of medications in the pediatric population, particularly in Hem/Onc and BMT.
- Ability to communicate effectively with diverse hospital personnel and patient groups.
- Capable of monitoring drug therapy and intervening as necessary regarding dosing and adverse effects.
- Skilled in interpreting medication orders and overseeing medication distribution.
- Adept at assessing patient conditions and evaluating responses to medication regimens.
- Ability to problem-solve and think critically.
- Provides supervisory oversight for technical staff involved in medication preparation and dispensing.
- Ability to respond promptly to issues, including weekends and holidays.
- Fosters cooperative relationships with patients, colleagues, and interdisciplinary teams.
- Familiarity with pharmacy systems and technology.
- Demonstrates motivation and accountability in professional practice.
