Key Responsibilities
The responsibilities of the Clinical Pharmacist (Hematology/Oncology) may encompass, but are not limited to, the following:
- Deliver advanced clinical services within a large healthcare facility, engaging in clinical pharmacy care practices that necessitate extensive clinical pharmacy knowledge, monitoring, communication, and evaluation skills.
- Develop a tailored therapeutic care plan based on individual patient data, which includes defining therapeutic endpoints, selecting suitable medications, and determining appropriate dosage regimens.
- Monitor and evaluate the therapeutic and adverse effects of medication therapy, including drug interactions, through the assessment of physical and laboratory parameters.
- Provide recommendations to enhance medication therapy, mitigate adverse drug events, and minimize medication errors while supporting the Pharmacy Service and Medical Center in analyzing drug use trends and risks.
- Conduct patient interviews (in-person, via telephone, or telemedicine) to gather medical histories and assess for potential drug interactions, polypharmacy, therapeutic duplication, noncompliance, dosing errors, and adverse drug reactions to ensure safe and effective medication use.
- Document patient medication histories and counsel them on the proper use of medications, emphasizing the importance of adherence to therapy.
- Act as a drug information resource, offering advanced knowledge and expertise to healthcare professionals regarding all aspects of patient medication therapy.
- Stay informed about the latest developments in hematology/oncology therapeutics and management.
- Participate in the education and training of pharmacists, pharmacy technicians, students, residents, and other healthcare personnel as necessary.
- Maintain professional competency by keeping abreast of new medications, therapies, innovations in hematology/oncology management, and pharmacy practice, integrating this knowledge into daily activities.
- Contribute to the creation and implementation of guidelines, audit systems, medication use evaluations, and data collection efforts to support quality assurance initiatives aimed at enhancing the safety, efficacy, and cost-effectiveness of medication therapy.
- Serve on medical center and/or VISN committees as assigned and communicate relevant information during staff meetings as appropriate.
- Coordinate and manage various hematology/oncology clinics, including video on demand, face-to-face, and telephone consultations.
- Utilize the VISTA/Computerized Patient Record System (CPRS) effectively.
- Adhere to VHA policies and procedures.
Travel Requirements: Occasional travel may be necessary to other facilities within the healthcare system.
