Position Summary
The Pediatric Clinical Pharmacist Specialist in Hematology/Oncology plays a crucial role in the Hem/Onc/BMT service by delivering patient care recommendations on medication therapy, assessing drug interactions, determining appropriate dosages, and optimizing treatment regimens to enhance patient outcomes while minimizing the risk of adverse effects. This role involves collaboration with a multidisciplinary team to ensure effective medication management for hospitalized pediatric patients facing hematologic and oncologic challenges, utilizing evidence-based practices and patient-centered approaches.
Key Responsibilities
- Conducts and documents medication therapy evaluations and interventions, focusing on dosing, administration, interactions, and monitoring of adverse reactions.
- Engages actively in clinical pharmacy programs and services, providing expert consultation in specialized areas.
- Participates in daily patient care activities, clinical meetings, and hospital committees, contributing to protocol development and quality improvement initiatives.
- Implements established policies and procedures to guarantee safe and effective medication therapies, offering therapeutic alternatives when necessary.
- Maintains documentation of quality activities to meet accreditation standards and regulatory requirements.
- Delivers drug information and educational support to pharmacy staff, clinical staff, medical residents, and patients.
- Keeps accurate records related to patient care activities.
- Instructs and mentors pharmacy students and residents on pediatric hematology/oncology and evidence-based pharmacotherapy.
- Develops and participates in quality improvement projects and medication use reviews.
- Prepares data for medication utilization evaluations and special projects as assigned.
- Demonstrates proficiency with pharmacy and clinical information systems.
- Functions as a Staff Clinical Pharmacist as needed, fulfilling essential duties for medication management.
- Contributes to cost-saving initiatives and assists in maintaining budgetary targets.
- Promotes the profession through presentations and community education.
Essential Requirements
- Bachelor's Degree in Pharmacy or Doctorate of Pharmacy.
- Current and valid Florida Pharmacist License.
- American Heart Association BLS certification.
- 4-7 years of experience in a hospital setting as a Pediatric Pharmacist or completion of a PGY-1 Pharmacy Residency in a Pediatric Environment or PGY-2 Pediatric Residency.
Knowledge, Skills, and Abilities
- Possesses specialized knowledge and/or completion of an ASHP-accredited residency program or advanced training in pediatrics and/or hematology/oncology.
- Exhibits strong analytical skills to research and evaluate drug information and resolve medication-related issues.
- Understands pharmacokinetics and pharmacotherapeutics relevant to the pediatric population, especially in Hem/Onc and BMT.
- Ability to communicate effectively with all levels of hospital personnel and patient groups.
- Capable of performing drug therapy monitoring and making necessary interventions.
- Skilled in interpreting medication orders and overseeing medication distribution.
- Adept at assessing patient care conditions and evaluating responses to medication regimens.
- Strong problem-solving and analytical thinking abilities.
- Ability to supervise technical staff in medication preparation and dispensing.
- Proficient in using pharmacy systems and technology.
- Self-motivated with a high degree of professional accountability.
